We are delighted to accept new patients living in our Practice area onto our list. Usually that includes people in the following post-codes W1, WC1 and WC2 and some parts of NW1 & SW1.
Register online
We are using an NHS online service called Register with a GP surgery that makes it easy to register with this GP surgery. This secure system will email us your registration form.
Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.
The service is designed and run by the NHS, so your personal information is safe. It reduces our administrative workload and makes it easier for you to register.
Paper forms are still available if you need one.

Register at our GP surgery with a paper form
‘Commuter Patients’ can register with us.
If you work in the locality and wish to register with a GP local to work rather than your place of residence then we are now able to register you as a patient here. You will need to provide a letter from your employer stating that you are employed nearby.
Medical Record
Your medical record will automatically be recalled from your previous Surgery when you register here. It may take up to 8 weeks for your paper record to arrive so we ask you to also complete a New Patient Health Form.
We ask all new patients to complete a questionnaire about their health to ensure that the Doctors and Nurse here have a brief personal history and an understanding of your health needs before your full record is available. Please give as much information as possible, and please include immunisation details for children. If you are not sure whether you have completed your forms correctly, please advise the Receptionist.
Keeping us up-to-date
Please let us know of any change in your name, address or telephone number. (especially mobile and email).
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill.
You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
Medical treatment is available from the date of registration. Please contact reception for further information.
The National Care Record Service (NCRS)
Important information regarding your NHS medical records – Summary Care Record
The National Care Record Service (NCRS) is the way in which health professionals now access the Summary Care Record (SCR). The SCR has not changed, it is just the way SCRs are accessed that has changed.
